Dry needling is an important part of the physical therapist scope of practice and there has been confusion in the past of how it should be coded and billed. One of the CMS proposed rulings for 2020 could possible clarify this situation.
The CMS is proposing two new CPT codes for dry needling. The new codes would be 205X1 and 205X2. CPT code 205X1 would have a relative value unit (RVU) of 0.32 with 10 minutes of intraservice work time, and a RVU of 0.48 for CPT code 205X2 with 15 minutes of intraservice work time.
In addition, CMS is also proposing to classify these two new codes (205X1 & 205X2) as “always therapy”. This means that they would count towards the annual therapy dollar threshold and multiple procedure payment reduction (MPPR) would apply.
